Die Integration zementfreier Hiiftendoprothesenschafte in das
knocherne Implantat lager mit daraus resultierender dauerhafter
Prothesenfixation lii6t sich im wesentli chen als Funktion der
Grenzflache zwischen Implantat und Knochen darstellen. Ais
Voraussetzung fUr eine funktionelle Anpassung des Knochenlagers ist
die primarsta bile Implantatverankerung anzusehen. Als Parameter
dieser Primarstabilitat konnen die am Implantat-/Knochen-Interface
auftretenden Mikrobewegungen herangezo gen werden, welche bei
Werten oberhalb der knochernen Toleranzgrenze das ange strebte
An-bzw. Einwachsen verzogern oder auch verhindern. Ais wichtige
Einflu6groge ist das Implantatdesign sowie die Oberflachenbeschaf
fenheit mitbestimmend fUr das Ausma6 der in der Verankerungszone
sich ausbilden den Mikrobewegungen, so dag durch biomechanisch
giinstige Designelemente des Prothesenschaftes die Voraussetzungen
fiir eine stabile biologische Schaftintegration optimiert werden
konnen. In einer biomechanischen vergleichenden Studie wurde zum
einen die Primarstabilitat eines vorwiegend proximal verankerten
zementfreien Prothesenschaftes iiberpriift und zum anderen eine
Designoptimierung des system kompatiblen, ausschlie61ich distal
verankerten Revisionsschaftes durchgefUhrt. Der dabei gewahlte
experimentelle Ansatz beinhaltete eine kombinierte Biege-und Torsi
onsbelastung, welche in Verbindung mit einer Dauerbelastung iiber
10 000 Lastzyk len sowie hochauflosenden optischen Wegsensoren eine
umfassende Beurteilung der jeweiligen initialen
Verankerungsstabilitat gewahrleistete. Durch die isolierte
Betrachtung einzelner Designelemente konnte eine in hohem Mage
stabilisierende Wirkung ventro-und dorsolateral angeordneter
Stiitzstege im FaIle der proximal ver ankerten Primarimplantate
ermittelt werden, wahrend fUr die insgesamt schwieri gere
Verankerungssituation der Revisionsschafte reduzierte
Setzbewegungen durch Designoptimierungen, wie z. B. die
Doppelkonus-Verankerung und eine zusiitzlich vorgesehene distale
